REQUIRED ITEMS:

1.   A completed Team On-Site Contact sheet (.pdf document obtainable from link on left hand menu of this page)

2.   Your team’s original certified roster along with two photocopies of the roster.

3.   Official player passes from your governing organization for all players.

4.   Signed medical releases for all players. The ’Medical Release Form’ pdf file on the link at left provides a form that you can use if you do not have existing medical releases from you team, club, league or state organization. All players must have a signed medical release substantially similar to that presented on the link at left.
